Overview

Clomiphene citrate is a drug that works by modulating estrogen receptors. It is used to control ovulation, and is available only on prescription.

The World Health Organisation considers cliomphene citrate to be an essential medicine. This means that it is a drug which they feel is one of the medications which must be available in even a basic health care system. It is used to treat infertility in women, although the evidence of its effectiveness in those who are infertile with ‘no clear reason’ is limited. It is known, however, that clomifene can treate oligoovulation and anovulation.

Even in women who do not suffer from either anovulation or oligoovulation, it has ben found that there is an improvement in pregnancy rates. Without treatment, women who are infertile for reasons that are still unclear tend to show fertility rates of 1.3 to 4.2 percent per cycle, and the fertility increases to 5.6 percent per cycle while they are being treated with Clomiphene citrate.

Typical Dosage

The typical dosage of Clomifene is 50mg per day, for five days, with the dosage starting on the third, fourth or fifth day of the menstrual cycle. For the purposes of this dosage, the first day of the cycle is considered to be the day when the woman’s menstrual period begins. The dose should run from day three to seven, or five to nine, depending on when the pills are started.

When a woman is on a clomifene-induced cycle, they are encouraged to have frequent intercourse – typically on alternating days, in the week before the expected day of ovulation, and also the week that ovulation is expected to take place. This means days nine through 18 of the cycle. The doctor may encourage LH tests, and these will be used three or four days after the final dose of Clomiphene citrate is given.

The tests will continue until it is inidcated that ovulation has happened. If there is no surge by day 18, then the doctor may opt to artificially trigger oocyte release, assuming that they know there is an ovarian follicle present that is greater than 20mm in size. This triggering will be done using an intramuscular injection of a large dose of hCG.

This will only be done in carefully selected cases, however, because routinely triggering oocyte release by using hCG has been found to reduce fertility. It is often better to simply monitor LH and attempt to find the most fertile period in this way.

Follicular monitoring is performed using ultrasound, and can help to reveal the number of developing follicles, as well as their size.
